More Access to Care. Same Family Doctor.

Beginning October 1, 2026, Dr. Jared Smiley, Dr. Daniel Freedman and Dr. Syed Hamid will be practising as part of the Toronto Central West Family Health Organization (FHO).

If you are an existing patient of one of these physicians, you may be asked to complete a Patient Enrolment and Consent form.

The most important thing to know:

Your family doctor is not changing.

You will continue to receive your regular medical care through The Clinic at Christie, book appointments with us as usual, and see your own family physician for your ongoing care.

Joining an FHO simply gives patients access to a larger team of family physicians and additional options for care when their regular physician is unavailable.


What is a Family Health Organization?

A Family Health Organization, or FHO, is a group of family physicians who work together to provide comprehensive primary care to their enrolled patients.

Your own physician remains your family doctor.

Being part of the FHO also gives enrolled patients access to additional services, including after-hours care.


Why should I enrol?

Enrolling allows you to remain formally connected to your family physician as part of the FHO and provides access to the additional services offered through the FHO.

This includes greater access to care when your regular family physician is unavailable and access to designated after-hours services.

There is no fee to enrol.
